To request an appointment online, please fill out the information below to help or staff set up an appointment that works best for you. Items on the form that are flagged with asterisks are required entries. If you find that you cannot keep your scheduled appointment, we ask that you provide a minimum of 48 hours notice so that we may schedule another patient in need of treatment. Failure to do so may result in a missed appointment fee of $30.00 for each scheduled hour or fraction thereof.
